I think in a lot of the images shown in that video the drastic change depicted was a result of weight loss, toning, stylist advice, air brush make-up, good photography, lighting, pose and photoshop. You could easily look at a crappy snapshot of me next to one of my portfolio images and say, "Oh. She's had work done."
It's all too easy to change someone's appearance completely digitally. See this link for an example:

Some of them have obviously had work done though and while it sucks that having the money to change your appearance generates more money and fame, I don't really care. It is pretty common practice these days and if people want to mutilate their faces that's their decision. I have rarely seen an after shot that looks better than the before though.
I think it is important though, for surgeons to take more responsibility in terms of psychiatric analysis and exercise more care in not operating on people with obvious conditions like body dismorphia syndrome (or whatever it's called) to prevent patients winding up looking like Michael Jackson.
